Once you schedule a Tax-Aide appointment, you can pick up a Pre-Appointment Packet from one of our Tax-Aide sites or download one by clicking the button below. You can also read it on this page by scrolling down.
All taxpayers must complete our Intake/Interview & Quality Review Sheet, Form 13614C which we often refer to as our Intake Booklet (click button below to download).
- If you are filing for a previous tax year, click here to access the Intake Booklet for that year.
- You will also need an additional appointment for each year’s tax return
Taxpayers should also print the Documents We Need (click button below to download) as it lists all the documents we need to prepare your return depending on your tax situation.
Some taxpayers will need to fill out additional worksheets. Visit our worksheets page for a full list of downloadable worksheets.
Our Intake Booklet and most worksheets are fillable forms that can be completed while viewing them on your computer or mobile device and then printed. If you prefer, you can simply print the forms and fill them out manually.
