Once you schedule a Tax-Aide appointment, you will be instructed to pick up a Pre-Appointment Packet from one of our Tax-Aide sites or told that you can download one here. 

All taxpayers must complete our Intake/Interview & Quality Review Sheet, Form 13614C which we often refer to as our Intake Booklet. Taxpayers should also print the Documents We Need as it lists all the documents we need to prepare your return depending on your tax situation. 

Taxpayers who are self-employed, who wish to itemize their deductions, claim an Education Credit, or who provided personal care services to a Washington State Department of Social and Health Services (DSHS) client, or had gambling winnings must also complete the appropriate worksheet and bring that to their appointment as well. 

If you are missing any documents when you arrive for your appointment, we may not be able to prepare your return.
